There are many ways to help out at Going Bananas, our July school holiday program:

Program team: Help engage the children with craft, Bible stories and songs. Training essential.

Registration: Welcome families, sign them in and help crunch statistics.

Kitchen: Feed the leaders and ensure the children are fed and watered.

Tech: Be involved with audio and visual.

Safety and first aid: Help keep the children and volunteers safe.

Car parking: Help keep things flowing in the car park.

Prayer: Sign up to pray for Going Bananas, either onsite or at home.

Ninja Banana: Problem solvers and helpers.

Mon July 7 10-2pm  Doing Day (optional craft preparation day)

 

Mon July 14 - Fri July 18 8:30-1:30pm  Going Bananas (WOO) 

Sun July 20  10am GB Sunday

 

In completing this form you commit to:

  • Attending all Going Bananas training sessions note - For Program team working with kids only
  • Complete all relevant child safe ministry requirements. 
  • You will be contacted about Child Safe requirements personally.

The data entered may be used to update your details in the Church Database. For example if you enter a new email address, then this will be used to contact you, church wide.

Please note you will need a Clearance for Service from Kooyoora (the Anglican Church's professional standards body) before you can serve.

This information will be passed on to kitchen staff so they can prepare snacks which is part of the program. 

Parent/Guardian and other contact information

If you are under 18 please have your parent fill out the following information.

If you have provided your child's email, both the child and parent 1 will receive all communications in regards to the Going Bananas Program. Otherwise just parent 1 will receive the communications.

If you are over 18 please complete this section of the form yourself.  If not, please ask your parent to fill this in.

Authorisation

I authorise the leaders in charge of any activity conducted by St Alfred's Anglican Church, Blackburn North, to consent on my behalf, where it is impractical to communicate with me or my nominated contacts, to receive medical or surgical treatment as may be deemed necessary. I am also responsible for the cost of any such medical treatment deemed necessary.

Indemnify

I agree to indemnify and hold harmless the Anglican Diocese of Melbourne and the church workers conducting this program against all claims, demands, suits and liability of whatever nature and howsoever arising out of injury of said leader and the relevant activity being undertaken.
